Dental Insurance

• You are fully responsible for all fees charged by this office regardless of your insurance coverage the day services are rendered.

• Ark Valley Dental is not an “in-network” provider with ANY dental insurance company. 

• Regardless of insurance carrier you may receive dental care by Ark Valley Dental as an “out-of network” benefit. You may be reimbursed by your dental insurance carrier at a lower reimbursement rate than if you were to receive care by an “in-network” provider. 
• It is important to understand that the insurance contract is between YOU (the insured) and the INSURANCE COMPANY.

• Our office will gladly submit your insurance claim to your insurance carrier, as a courtesy to you.
